The Health & Wellness Center (HWC) has been providing quality healthcare services to our communities since 2005. As a Community Health Center (CHC), we offer medical, dental, behavioral health, pharmacy, and optometry services to those with private insurance, Medicare, Oklahoma Medicaid/SoonerCare, as well as the uninsured/under-insured, with fees based on household size and income. We currently have seven locations in Eastern Oklahoma. In 2014 HWC had a total patient population of almost 23,000 with over 80,000 patient encounters. HWC is now the 2nd largest Community Health Center (CHC) in Oklahoma and the LARGEST of all rural Oklahoma CHC's. We will continue to strive to become the clinic of choice for all area residents, focusing on becoming Patient Centered Medical Homes for many and providing quality care to all.
