Our mission is to improve the health of our patients by providing quality health care and supporting comprehensive community-based programs.
HHC is a federally qualified health center, accredited by the Joint Commission, and located in a Medically Underserved Area (MUA) and Health Professional Shortage Area (HPSA). As the only free-standing primary comprehensive health care center in the greater Holyoke and Chicopee area, our work is focused on eliminating the health disparities experienced by local populations. . HHC has been the only free-standing comprehensive health center in the greater Holyoke area with an onsite 340B pharmacy and integrated oral health and behavioral health services. Culturally competent programming, an active referral network, bilingual patient services and care management have been in place for many years that evolve as needed to better serve our patients and community. Our most current goals include our transformation into a comprehensive patient-centered medical home (PCMH) and our conversion to electronic health records to ensure higher quality and more accessible health care.
