A Patient Centered Medical Home (PCMH)
The Cordelia Martin Community Health Center was started in 1969 by a group of community residents who wanted to give access to healthcare to inner city residents who did not have the means to pay for their healthcare, who held jobs for little pay, and supported families as best as they could. This concern group of residents lead by Mrs. Cordelia Martin brought their voices to mobilize and be heard.
