Main Site

The Center's main site is located at 1205 Broadway in a health center-owned building that was recently renovated and offers service expansion opportunity. LCH&D owns and operates a satellite site at 3745 Grove Avenue, Lorain where adult medicine and podiatry services are provided. Our newly renovated main sites serves pediatrics, women's health, dental and vision services. We have 2 locations in nearby Elyria, OH as well, serving pediatrics and adults. We opened our 5th site in Oberlin in April, 2016. We also serve adult patients in nearby Amherst, OH. The state-of the-art optometry service opened in October 2013 to meet the vision needs of all persons, especially those with chronic illnesses (such as diabetes and hypertension) who are more prone to sight impairments. LCH&D recently achieved recognition by the National Committee on Quality Assurance as a Level 3 (the highest level) Patient Centered Medical Home for its three eligible sites (Broadway, Grove and East River).
