Banyan Community Health Center is a Federally Qualified Health Center and an NCQA-Certified Patient Centered Medical Home. Our purpose in the communities we serve is to advance health and well-being of our patients and their families through improved integration of prevention, disease control, and evidence-based clinical services in behavioral and primary health care. We are proponents of family-centered, culturally effective Health Homes, a healthcare strategy characterized by interdisciplinary clinical teams, comprehensive and fully integrated health services, and meaningful use of health information. A growing base of research and evidence-supported practices tells us that these health care strategies accelerate the progress of people with chronic diseases, including diseases of the brain, improving their physical health and well-being while increasing their ability to manage the activities and challenges of everyday life.
